Terms and Conditions for Horizon Holistics Pty Ltd

Thank you for choosing Horizon Holistics Pty Ltd as your preferred physiotherapy provider. By booking or attending an appointment with us, you agree to the following terms and conditions. Please read them carefully.
1. Cancellations
1.1. If you need to cancel your appointment, please notify us as soon as possible by calling [Insert Clinic Phone Number].
1.2. Appointments remain confirmed unless cancelled with at least 48 hours' notice.
1.3. If an appointment is cancelled with less than 48 hours' notice, the full consultation or service fee is payable by you. We reserve the right to invoice you for the full fee and may decline future service requests.
2. Late & Missed Appointments
2.1. If you arrive late but within your appointed time slot, your consultation may be shortened to avoid delays for other patients. The full consultation fee remains payable, regardless of its length due to your late arrival.
2.2. If you arrive after the appointment time or miss an appointment, you may be charged the full consultation fee.
3. Consent to Treatment
By accepting these terms and conditions, you:
3.1. Consent to necessary examinations, manipulations, therapy, rehabilitation, and medical diagnostic procedures by our therapists based on their professional training and understanding of your injury.
3.2. Have the right to discuss the nature and purpose of your physiotherapy care before any treatment is rendered.
3.3. Acknowledge and accept the risks associated with physiotherapy, such as stiffness, soreness, neurological complications, and other minor complications.
3.4. Understand that delay in treatment may complicate the condition and future rehabilitation.
3.5. Agree that most clients do not require a medical or other referral for our services, but if you have one, we recommend providing it to ensure the most appropriate treatment.
4. Consent to Obtain and Release Information
4.1. Acknowledge that Horizon Holistics Pty Ltd may need to communicate your medical condition and treatment details to necessary third parties, including treating practitioners, rehabilitation consultants, case managers, employers, and insurers.
4.2. Permit Horizon Holistics Pty Ltd to obtain and release information related to your medical condition and treatment with third parties, as necessary.
5. Accounts and Billing
5.1. Our fees are outlined in our schedule of fees, available upon request.
5.2. Full payment is required at the time of consultation, and we accept cash, direct deposit, EFTPOS, and credit cards.
5.3. Health insurance rebates (electronic or otherwise) can only be claimed after receiving the service.
5.4. In cases of financial hardship, you may request individual consideration by writing to [Insert Clinic Email Address].
6. Exclusion of Liability
Horizon Holistics Pty Ltd and its employees are not liable for any injury, loss, or damage arising from treatments or procedures unless arising from any negligent act or omission. We do not accept responsibility for injuries or damages not resulting from negligence.
